Case Manager (LCSW) with +5 years experience

Use your expertise to help develop and operate a brand new one on one parenting concierge service (including multidisciplinary evaluation and case management) for parents and children 0-6 years old in the Bay Area. 


The case manager will assist with planning and execution, contribute ideas to shape the service, identify and implement best practices and procedures, and take initiative and ownership to ensure the service’s success. They will also be responsible for evaluating new and prospective clients based on needs, goals, and constraints and for monitoring cases from start to completion.

Responsibilities and Duties 

  • Works with clients that include both children and adults from diverse backgrounds with varied social identities to assess client needs and develop strategies to meet those needs, providing appropriate support and resources

  • Gathers clinical information from prospective patients, family members, and referring clinicians; financial, legal, and medical data; and any additional pertinent information to determine appropriateness of referral plan

  • Disseminates significant information to team members for diagnosis, coordination, and planning

  • Provides psychosocial assessments and develops case formulation with recommended viable care and prevention plans using Family Systems or Ecological Systems lens

  • Educates parents through 1:1 meetings or workshops and webinars

  • Researches, evaluates, and studies programs, services, and resources like health and childcare for quality and efficacy

  • Determines the appropriate course of action when helping clients (i.e. providing a referral to an occupational therapist, psychologist, medical doctor, family social worker)

  • Maintains good working relationships with support networks, government resources, and community resources, influencing community members and policymakers to develop programs, policies, and services that assist with social work

  • Advocates and raises awareness on behalf of clients, the needs of the community, and local services

  • Follows all legal and ethical guidelines in the execution of duties as outlined by the National Association of Social Workers.

Qualifications and Skills 

  • Master’s degree in Social Work from an accredited school of social work required

  • Valid LCSW license in accordance with CA requirements 

  • At least 5 years of experience working directly with children and families; at least a significant portion of that time working with children 0-12, mental health, or neurodiversity 

  • Has worked with children and families in the Bay Area 

  • Excellent interpersonal skills with colleagues, community leaders, policymakers, healthcare professionals, and clients

  • Superb written and spoken communication skills in English (other languages a plus)

  • Enjoys working in multidisciplinary teams

  • Demonstrated skills in diplomacy and interpersonal sensitivity, with an ability to identify and maintain boundaries

  • Able to assess children and families holistically, with a balanced view of strengths and areas for improvement 

  • Excellent time management skills; organized and able to prioritize

  • Motivated to identify gaps and issues and suggest solutions 

  • Able to take instruction from multiple sources, take initiative, and follow through to ensure effective outcomes

  • Comfortable in a fast-paced environment with multiple cases

  • Excellent organizational skills and able to manage large amounts of files, schedules, dates, and information

  • Self-directed and able to work without supervision

  • Comfortable with building personal relationships and dedicated to helping others

  • Empathetic and supportive with mentorship and leadership skills

  • Proficient computer skills, including Microsoft Office Suite (Word, PowerPoint, and Excel); comfortable learning new software 

  • Able to travel locally to meet with clients
